Notification of Birth・Death

You must submit this document after learning of someone’s passing.

Family, those living in the same household, the head of the household, the landowner, house or land manager, director of a public office, or guardian.

Within 7 days of learning of the death.

If you are a foreign national, please contact the appropriate embassy for your area and nationality as soon as possible. They can help you in completing any necessary procedures.

All of the following.

• Notification of Death (Shibou-Todoke/死亡届)
• Death Certificate (Shibou-Shindan-sho/死亡診断書) created by a hospital (right side of Notification of Death)
• Personal Seal/Hanko stamped in the “Todokede Ninran/届出人欄” section
• If a guardian is applying, a Touki Jikou Shoumei-sho/登記事項証明書, OR a Shinpan-sho Touhon/審判書謄本 AND a Kakutei Shoumei-sho/確定証明書

Submit the Notification of Death to the municipal office of the deceased's registered domicile, place of death, or the location of the applicant.

You must submit this document when a child is born.

The child’s father or mother.

Within 14 days from when the child is born, counting the day of birth as the first day.

All of the following.

• Notification of Birth (Shussei Todoke/出生届)

• Birth Certificate (Shussei Shoumei-sho/出生証明書) created by a hospital (right side of Notification of Birth)
• Personal Seal/Hanko stamped in the “Todokede Ninran/届出人欄” section
• Mother and Child Health Handbook (Boshi Kenkou Techou/母子健康手帳)

Submit the Notification of Birth to the municipal office of the parent's registered domicile, the place of birth of the child, or the location of the applicant.
