National Health Insurance
In the following cases, please be sure to report to city hall within 14 days.
- Your company does not take care of the procedures for joining or withdrawing from National Health Insurance. Please be sure to complete the procedure yourself.
- A power of attorney is required if someone other than the person in question comes to complete the procedure.
- Identity verification documents are documents issued by public offices and have a photo to confirm your identity. (Example: Basic Resident Register Card, driver's license, passport, etc.)
Joining the National Health Insurance System

(NOTE1) A certificate stating the date of disqualification of health insurance for all members is required. You cannot use items that do not have the social insurance number and date of loss, such as a turnover slip.
Leaving the National Health Insurance System
