Individual Number Card (My Number Card)

An Individual Number Card (A.K.A. My Number Card) can be used as a form of ID that includes your photo, address, full name, birthdate, gender, and your individual identification number.

It can also be used for electronic applications for national taxes (e-TAX) by using the IC chip on the card, and is expected to have many more applications in the future.

The issuance of an Individual Number Card is free initially, but there will be a fee for reissuance if the card is lost or damaged, etc.

Individual Number Card Application Process

In order to apply for the Individual Number Card, please check out the following steps.

Step One

Receiving application form for Individual Number Card in mail

Receive Application Form

  • After October 2015, the notification card of my number is sent by registered mail to the address on your resident card (for persons who have evacuated due to the Great East Japan Earthquake and nuclear power plant accident, DV victims, etc., the home address that they reported to the city).

Step Two

Example of Individual Number Card application

Paste a photo of your face onto the enclosed “Individual Number Card Issuance Application Form” and submit it using methods 1 ~ 4.

  1. Send the application form via the provided envelope.
  2. You can also scan and use the QR code provided to upload a photo and complete the application.
  3. Submit the application form at the office of the municipality in which you reside. (Note: Persons who have evacuated due to the Great East Japan Earthquake and nuclear power plant accident, DV victims, etc.)
  4. Go to city hall and complete the application form there.

You will be issued your Individual Number Card via the Civic Service Division.

If you choose method 3 or 4, you must bring...

  • Individual Number Card Application Form
  • ID Photo (no hat, no background)
  • Documents proving your identification (ex; passport, residence card, etc.)
  • Notification Card
  • Basic Resident Register Card (holders only)

Step Three

Your Individual Number Card will be sent to city hall. Afterwards, it will be issued to you via the following methods:

If you applied via method 1 (mail) or 2 (smartphone), you will be sent a delivery notice in the mail. Fill out the necessary information and go to the nearest city hall or ward office bringing the necessary documents below.

If you applied via method 3 or 4, your Individual Number Card will be sent to you in the mail.
