List of when you need to report and what you need to bring

You need to report to city hall when... Necessary Documents
You quit your job. Pension booklet, personal seal/hanko, documents proving the date of which you are leaving your place of employment (ex; a certificate of disqualification, etc.).
When you turn 20 years old and need to join the national pension system. Personal seal/hanko, Shikakushutoku-Todokedesho(資格取得届書).
When you lose financial support from your spouse. Pension booklet, personal seal/hanko, documents that show when you lost financial support from your spouse (ex; certificate of disqualification, etc.).
When your address or name changes. Pension booklet, personal seal/hanko.
When you lose your national pension booklet/certificate. Personal seal/hanko.
When requesting national pension. Pension booklet, personal seal/hanko, bank passbook or family register, and so on, in the name of the person requesting the pension.
When a pensioner dies. Pension certificate, personal seal/hanko, certificate of residence, family register, etc.
