Pregnancy Notification (Mother and Child Health Handbook)

A mother and child health handbook will be issued to those who have submitted a notification of their pregnancy.

We recommend that you report early (up to the 11th week of pregnancy) in order to receive subsidies for the maternity health checkups.

Ninsanpu Kenkou Shinsa Jushin-hyou/妊産婦健康診査受診票, Ninpu Shika Kenkou Shinsa Jushin-hyou/妊婦歯科健康診査受診票, Shinseiji Choukaku Kensa Juken-hyou/新生児聴覚検査受検票 are included.

Those who have a registered address within Minamisoma and had their pregnancy confirmed through a medical institution.

Personal seal/hanko, Individual Number Card (My Number Card) or the notification card (Tsuuchi Kaado/通知カード), driver’s license (something to verify your identification), etc.

A reservation is required to visit the Odaka Health and Welfare Center and Kashima Health Center.

Public health nurses and midwives provide consultation so that pregnant women can give birth and raise children with peace of mind, so please make sure that the mother-to-be visits the office if at all possible.

If you are unable to come to the office due to unavoidable reasons such as poor physical condition, the documents can also be issued through a legal representative. However, power of attorney is required. 

1. The pregnancy notification form, questionnaire, and power of attorney that has been filled out by the mother. (The power of attorney document can be downloaded below)
2. Identity verification documents for the legal representative (My Number Card, driver’s license, etc.)
3. Individual Number verification documents for the mother (mother’s My Number Card, notification card, etc.).
