Early Identification System for Wandering Elderly

The city has implemented an “early identification system for wandering elderly” so that when an elderly person who has disappeared due to conditions such as dementia is found by the police, etc., the people who found them can contact family members and supporters by using a dedicated QR code used to confirm their identity at an early stage.

Use of this service, distribution of the dedicated QR code sticker and QR code iron-on sticker are free, but an application is required.

We will issue one sheet with a total of 36 stickers ranging from large, medium, and small sizes. The sticker is waterproof and can be attached to sticks, walkers, wallets, shoes, etc.

The QR code is a registered trademark of Denso Wave Incorporated.

Dedicated QR Code Iron-On Sticker

As with the dedicated QR code stickers, we will issue one sheet with a total of 34 stickers ranging from large, medium, and small sizes. The iron-on sticker can be attached to fabric products. It can be used for clothes and cloth bags.

Application Form

Please submit the application to the Longevity Welfare Section at city hall or the Welfare Section of either ward office.

It is also possible for the Regional Comprehensive Support Center or the primary caretaker to submit an application on behalf of the elderly. Please contact us to learn more.

Things required for the application process: applicant, application, and applicant’s personal seal/hanko.
