About the Medical System for the Elderly

Since April of 2008, the “Medical System for the Elderly” started as a new medical system for elderly persons aged 75 years and older.

How to Join

Since you become automatically enrolled into the medical system for the elderly once you turn 75, there aren’t any necessary procedures to complete in order to join it. Health insurance cards will be delivered to your home by mail about 1-2 weeks before your 75th birthday.

In addition, those aged 65 to 74 who have certain disabilities can join the medical system by submitting an application form to the association. For more details, please contact the section in charge.

About the User Burden

The burden required to pay at the counter of the hospital is 10% of the medical expenses incurred.

 However, it will be 30% for those who have income above a certain level.

About the Calculation of Insurance Fees

Annual insurance premium = Per capita rate (fixed amount) + Income rate (amount according to the insured's income)

  • (NOTE) The maximum annual insurance premium is 640,000 yen.
  • (NOTE) Insurance premiums are rounded down to the nearest 100 yen.

The insurance premium rates for the 2nd and 3rd fiscal years of Reiwa are as follows (the per capita rate and income rate are reviewed every two years).

About Reductions of Insurance Premiums

Reduction of Per Capita Rate

For low-income households, there are insurance premium reduction measures. The per capita rate (43,300 yen) will be reduced according to the total income of the head of household and the insured.

20% (8,660 yen decrease) Less than 330,000 yen + (520,000 x number of insured persons)

(NOTE) Pension income for people aged 65 and over (as of January 1st) will be determined by deducting a special deduction (150,000 yen).

Reduction of Income Percent

There will be no reduction measures for income percentage from 2018 onward.

Reduction for Employee Insurance Dependents

Those who have transitioned from dependents of employee insurance into the medical system for the elderly aged 75 or over will only be charged 50% of the per capita rate (43,300 yen), and will not be charged the income rate.

As a result, the annual insurance premium will be 21,600 yen.

How to Pay Insurance Premiums

As a general rule, insurance premiums are specially collected from the pension. However, the following people will be required to pay by direct debit or payment slips.

  • Those who receive a pension amount of less than 180,000 yen (annual amount)
  • Those whose total amount of medical insurance premiums for the elderly and long-term care insurance premiums exceeds half of the received pension amount

About Changing Insurance Premium Payment Methods

Among those who have paid the insurance premium from their pension, those who wish to pay by bank transfer can request to change it.

Failure to Pay Insurance Premiums

If you fail to pay your insurance premiums without special reason, you will be issued a short-term insured card (Tanki-Hihokensha-shou/短期被保険者証) with a short expiration date.

 In addition, if you fail to pay insurance premiums for one year or more without any special reason, you may be issued a "qualified certificate" (Shikaku-Shoumeisho/資格証明書), and if you visit a medical institution, you will be responsible for the full amount.
