Adult Diapers and Nursing Care Products Subsidy Project

The city will subsidize part of the cost of purchasing disposable diapers, etc. for families who are caring for the elderly who are bedridden or have dementia at home.

Changes of purchasable items and the application form format (from April 1st, 2019)

With the revision of the Minamisoma City Diapers and Nursing Care Product Subsidy Project Implementation Guidelines, the items that can be purchased, usage vouchers, application forms, etc. have changed. The application form is posted at the bottom of this page.

About the Subsidy

Those who have a certificate of residence (Juumin-Hyou/住民票) and apply to one or more of the following will qualify for the subsidies.

Up to 3,000 yen per month (1,000 yen voucher x 3)

Issued up to 36,000 yen per year

Family members who are currently caring for a person who falls under any of the following at home:

(A) Persons aged 65 and over who are bedridden and are receiving long-term care at home

・ Bedridden degree: A1 or higher

(B) Persons aged 65 and over who are receiving long-term care at home and have dementia

・ Dementia: IIa or higher

A visit survey by a care manager or the staff of the Regional Comprehensive Support Center is required to determine the degree of bedridden status and dementia.
Nursing Care Product Subsidy Vouchers
Subsidy Info Who Can Apply

Up to 10,000 yen per month (1,000 yen voucher x 10)

Issued up to 120,000 yen per year

Family members who are exempt from municipal tax and are currently caring for persons (including a considerable number of people) who are certified as requiring long-term care level 4 or 5 at home.

All household members, both those who are caring and those who are receiving care, must file a tax exemption declaration. Those who do not file a tax exemption declaration are considered taxable and are not eligible.
  • If you are currently evacuated from the city, please purchase disposable diapers, etc., and then make a request to the city with a receipt in order to receive the subsidies.
  • Disposable diaper tickets and long-term care products vouchers cannot be issued more than once.
  • Vouchers will be issued on the first day of the month following the issuance decision.
  • Vouchers cannot be used for any month other than the stated month and you cannot purchase products for amount lesser than what it is worth. Please be sure to use it when paying 1,000 yen or more.
  • Even if the vouchers are lost, it cannot be reissued, so please handle it with care.

The vouchers have changed

From April 1st, 2019, the items that can be purchased, usage vouchers, application forms, etc. have changed. The main changes are listed below.

  • The number of items eligible under the subsidies for disposable diapers and nursing care products has increased. Please check the further for more details.
  • Vouchers will be changed from 3,000 yen or 4,000 yen to 1,000 yen each. (There is no change in the subsidy amount.)

Purchasable products using the vouchers

The disposable diapers and nursing care products that are able to be purchased are as listed below. (The amount of items eligible under the subsidies has increased starting from April 1st, 2019.)

  • Disposable diapers
  • Feminine napkins
  • Disposable gloves
  • Cleaning Products
  • Dry shampoo
  • Wipes
  • Oral care brushes (口腔ケアブラシ)

Please see the attached document for a list of registered stores in Minamisoma that accept the use of the disposable diaper and nursing care product subsidy vouchers.

Situations where you may have to return your vouchers

If the person receiving long-term care falls under any of the following, please return the vouchers immediately.

  1. When they move outside of the city.
  2. When they pass away.
  3. When you no longer use disposable diapers or nursing care products.
  4. When they are hospitalized and there is no possibility of them being discharged.
  5. When they are admitted into a facility. (NOTE)

(NOTE) The facilities in number 5 refer to long-term care insurance facilities (special nursing homes for the elderly, long-term care health facilities, or long-term care medical facilities), and other facilities with long-term care services.

If you have returned your vouchers at the time of admission to the hospital or facility and the conditions for using the vouchers are met again during the same year due to discharge, etc., please contact us. We will issue the vouchers again.

The application method is only through proxy application by the Regional Comprehensive Support Center or the care manager. Please see the table below for details.

Application Forms

From April 1st, 2019, the application format has changed. When applying for the subsidy, please use the updated application form.

These application forms can be used by the Regional Comprehensive Support Center or the chief care manager.
