Mental Illness Certificate Application Process
Based on the prefectural regulations, the city accepts applications for the Mental Illness Certificate.
The Mental Illness Certificate proves that the recipient has a certain mental disability.
Having the certificate makes it easier to receive various kinds of support, and is intended to help the recipient by offering support for independent living and social participation.
The Mental Illness Certificate is issued by the prefectural governor via the Mayor of Minamisoma and the application window.
It takes about two months from applying for the certificate to issuance.
Who Can Apply
The certificate is intended for those who have been affected by mental disabilities for a long period of time and have difficulties in daily life and social participation.

Validity Period
The validity period of the Mental Illness Certificate is 2 years from the day it is issued.
You can apply to have your certificate renewed up to 3 months in advance.
City hall does not send out notices for renewals.

- In case of re-issuance due to loss or damage.
- If your address or name changes.
- If you move.
- If you die.
