Submitting Notification of Moving Out by Mail

As a general rule, Notifications of Moving Out are submitted at a city hall counter in person, but if you have already moved out of Minamisoma and cannot come to city hall, you can submit your Notification of Moving Out by mail.

If you do submit the notification by mail, the Certificate of Moving Out (転出証明書) will be mailed back to you by Minamisoma, so please complete the moving in process at the town hall in the municipality in which your new address is located.

After applying, it may take approximately a week for your Certificate of Moving Out to be delivered to you, so please make sure you have enough time when submitting your notification. However, if you submit a Special Notification of Moving Out using your My Number Card or Basic Resident Register Card, you will not be issued a Certificate of Moving Out. Once Minamisoma has processed your notification, you will receive a phone call, so once you are called please be sure to use your card for the moving in process.

What to Send

Please send all of the following:

1. Mail Notification of Moving Out

Please write all of the following on the “Mail Notification of Moving Out and Request for Certificate of Moving Out” found below, or download the PDF and input the information.

  • Notifier’s full name
  • Phone number at which you can be reached during the day
    (Note) You may be contacted to confirm the information, so please be sure to fill in a phone number.
  • Date of transfer (the day you moved to your new address)
  • New address (including apartment/building name and room number)
  • Head of household at new address
  • Address in Minamisoma (including apartment/building name and room number)
  • Head of household at Minamisoma address
  • Full name(s), birthdate(s), and connection to head of household for all members moving

2. Identity Verification Document of Notifier (Copy)

Please send a copy of the notifier’s ID in the same envelope as the notification.

  • If choosing from Category 1 (government-issued photo ID), send one document.
  • If choosing from Category 2 (government-issued ID without photo), send two documents

3. Return Envelope

Please write your name and return address on a return envelope, attach a stamp worth 84 yen (if using a standard-size mail envelope), and put it in the same envelope as the notification and the ID.

For the return address, you can only use the address to which you are moving or your Minamisoma address. Please be aware that we cannot send the return envelope to your workplace, temporary residence, or any other location.

The return envelope will be used to send your Certificate of Moving Out. If you are submitting a Special Notification of Moving Out using your My Number Card or Basic Resident Register Card, you will not be issued a Certificate of Moving Out, so you do not need to include a return envelope.
